Reportable Conduct Notes

Learn how to add official case notes, classify note types, and record internal updates, findings, risk notes, and management notes for a case.

Before You Start

You need access to Child Safety and Reportable Conduct.

A reportable conduct case must already exist before you can add notes.

Case notes are permanent and form part of the official case record.

Open the case workspace

From the left sidebar, open Child Safety.

Select Reportable Conduct.

Open the case you want to update.

This opens the RCS Case Workspace.

Open the Notes tab

At the top of the case workspace, select Notes.

The Notes tab shows the Case Notes section.

If no notes have been added yet, the page will show No case notes.

Add a note

Select Add Note.

You can use the Add Note button on the right side of the Case Notes section or the button in the empty state.

This opens the Record Case Note modal.

Select a note classification

Use the Classification dropdown to choose the type of note being added.

Available options include:

  • Internal Note

  • Risk Note

  • Finding

  • Investigation Update

  • Management Note

Choose the classification that best matches the note content.

Add the note content

In the Content field, record the details of the note, observation, update, or finding.

Keep the note factual and relevant to the case.

Examples of note content may include:

  • A case review update

  • A risk-related observation

  • A management decision

  • An investigation progress update

  • A finding from the case review

Record the note

After selecting a classification and adding content, select Record Note.

The note will be added to the case notes register.

If the Record Note button is disabled, check that the required fields have been completed.

Important Notes

  • Notes are permanent and form part of the official case record.

  • Use the correct classification so notes are easy to review later.

  • Do not include unnecessary personal opinions.

  • Use clear, factual wording that supports the case record.

  • The Notes tab is separate from the Timeline and Audit tabs. Notes are written case updates, while Timeline and Audit show activity history.

Troubleshooting

  • If the Record Note button is disabled, make sure a classification and content have been added.

  • If the note does not appear after saving, refresh the page and check the Notes tab again.

  • If you selected the wrong classification, add a new note with the correct classification if editing is not available.

  • If you cannot add notes, check your permissions for Reportable Conduct.

  • If the Notes tab is empty, no notes have been recorded for the case yet.
