Reportable Conduct Forms

Learn how to open, complete, save, and mark official reportable conduct forms as complete from the case workspace.

Before You Start

You need access to Child Safety and Reportable Conduct.

A reportable conduct case must already exist before you can access official forms.

Forms are used to support regulatory and compliance steps for the case. Some forms may be completed at different stages of the case workflow.

Open the case workspace

From the left sidebar, open Child Safety.

Select Reportable Conduct.

Open the case you want to manage.

This opens the RCS Case Workspace.

Open the Forms tab

At the top of the case workspace, select Forms.

The Forms tab shows Official Forms for the case.

Review the available forms

Each form appears as a card with a form number, title, short description, and status.

Examples include:

  • Form 1: Safeguarding Allegation Intake

  • Form 2: Regulatory Threshold Decision

  • Form 3: Safeguarding & Workforce Risk Plan

  • Form 4: Head of Reporting Entity Attestation

  • Form 5: Commission Initial Notification

  • Form 6: NDIS Reportable Incident Notification

  • Form 7: Letter of Allegation

  • Form 8: Investigation Plan

  • Form 9: Interview Record

  • Form 10: Investigation Findings Report

  • Form 11: Worker Screening Review

  • Form 12: Reportable Conviction Verification

A form may show a status such as Not Started.

Open a form

Select the form you want to complete.

The form opens in a document-style view.

The form includes case details at the top, such as the case reference, date aware, date of alleged conduct, and prepared date.

Complete the form fields

Complete the fields shown in the form.

Depending on the form, fields may include:

  • Toggle questions

  • Checkbox options

  • Date fields

  • Time fields

  • Text fields

  • Sign-off fields

  • Decision fields

  • Rationale fields

For example, Form 1 may include:

  • Immediate Safety Assessment

  • Immediate Risk Controls

  • Criminal Threshold

  • Escalation Record

  • Intake Officer Sign-off

Save the form as a draft

Select Save Draft if you are not ready to finish the form.

Use this when you need to return later, collect more information, or confirm details before completion.

Mark the form complete

Select Mark Complete when the form is finished and ready to be recorded as complete.

Only mark a form complete once the details have been reviewed and are accurate.

Close the form window

Select Close Window when you are finished reviewing or editing the form.

This returns you to the case workspace.

Open another form if needed

Return to the Forms tab and select another form.

For example, Form 2: Regulatory Threshold Decision may include sections such as:

  • RCS Threshold

  • CSSC notified

  • NDIS Threshold

  • Restrictive Practice Check

  • Worker Screening Review

  • Sign-off

Complete each form based on your organisation’s reporting process.

Important Notes

  • Forms are connected to the selected reportable conduct case.

  • Some case details may automatically populate into the form.

  • Each form supports a specific reporting or compliance step.

  • Use Save Draft when the form is incomplete.

  • Use Mark Complete only when the form is ready to be finalised.

  • Some forms may require sign-off fields before completion.

Troubleshooting

  • If a form does not open, refresh the case workspace and try again.

  • If Mark Complete is unavailable, check whether all required fields have been completed.

  • If case details look incorrect in the form, review the case details in the Overview tab.

  • If you need to pause your work, select Save Draft before closing the form.

  • If you cannot access forms, check your permissions for Reportable Conduct.
