Reportable Conduct Reports

Learn how to generate, preview, and review Initial, Interim, and Final Reports for a reportable conduct case.

Before You Start

You need access to Child Safety and Reportable Conduct.

A reportable conduct case must already exist before you can generate reports.

Reports are created from the current case data. Make sure the case details, involved parties, response actions, files, notes, and risk actions are up to date before generating a report.

Open the case workspace

From the left sidebar, open Child Safety.

Select Reportable Conduct.

Open the case you want to manage.

This opens the RCS Case Workspace.

Open the Reports tab

At the top of the case workspace, select Reports.

The Reports tab shows report types available for the case.

Choose a report type

Use the report list on the left to select the report you want to review or generate.

Available report types include:

  • Initial Report

  • Interim Report

  • Final Report

Each report type has its own purpose and reporting requirements.

Review the Initial Report

Select Initial Report.

This report is used as an initial notification to the relevant oversight body. It includes the nature of the allegation, the individuals involved, and any immediate actions taken to manage risk.

The page shows the statutory requirement and report record.

The report record may include:

  • Status

  • Deadline

  • Case Reference

  • Generated

  • Generated by

  • Submitted

  • Submitted by

  • Snapshot ID

  • Email Confirmation

Review the Interim Report

Select Interim Report.

This report provides a progress update on the investigation, including interim findings, ongoing risk management measures, and any changes to the circumstances of the matter.

The page shows the statutory requirement and the report record.

Review the Final Report

Select Final Report.

This report provides a comprehensive account of the completed investigation, including the final determination, disciplinary outcomes, evidence relied on, and any systemic improvements implemented.

Generate a report

Select Generate Report.

A confirmation modal will appear.

The modal explains that the report will create a point-in-time snapshot of the current case data. The snapshot is assigned a unique ID for tracking purposes.

You can choose:

  • Generate report
    Creates a tracked snapshot with a unique ID and generates the official report.

  • Preview report
    Downloads a PDF of the current case data without creating a tracked snapshot.

Choose Generate report or Preview report

Select Generate report if the report should be officially created and tracked.

Select Preview report if you only want to download and review a PDF before creating the official report.

Then select Generate.

A PDF will download after the report is generated or previewed.

Review the downloaded PDF

Open the downloaded PDF and check the report details.

The PDF may include:

  • Case reference

  • Classification

  • Risk level

  • Date became aware

  • Report date

  • Prepared by

  • Document ID

  • Case details

  • Involved parties

  • Immediate response

  • Supporting case information

Confirm the PDF details are correct before submitting or using the report externally.

Mark a report as submitted

After a report has been generated, the page may show Mark as Submitted.

Use this when the report has been submitted through the required process.

This helps keep the report record updated.

Important Notes

  • Reports are generated from the current case data.

  • Use Preview report if you want to check the PDF before creating an official tracked snapshot.

  • Use Generate report when the report is ready to be recorded with a unique snapshot ID.

  • The Initial Report, Interim Report, and Final Report each have different reporting purposes.

  • The report record helps track whether a report has been generated or submitted.

  • Make sure case information is accurate before generating the official report.

Troubleshooting

  • If Generate Report is not available, check your permissions for Reportable Conduct.

  • If the PDF has missing or incorrect details, update the case information first, then generate or preview the report again.

  • If the report does not download, check your browser download settings and try again.

  • If the report record does not update after generating, refresh the page.

  • If Mark as Submitted does not appear, confirm that an official report has been generated.

  • If you only need to review the report, use Preview report instead of Generate report.
