Add a Reportable Conduct Case

Learn how to create a new reportable conduct case by adding matter details, involved parties, location information, and response actions.

Before You Start

You need access to Child Safety and Reportable Conduct.

A reportable conduct case is used to record and manage a matter that may require statutory reporting or internal review.

Required fields are marked with an asterisk.

Open Reportable Conduct

From the left sidebar, open Child Safety.

Select Reportable Conduct.

This opens the Reportable Conduct case list.

Start a new case

Select New Case.

If there are no cases yet, select Create first case.

This opens the New Reportable Conduct Matter workflow.

Complete Matter Details

In Step 1: Matter details, add the core information about the matter.

Complete the available fields:

  • Matter Title

  • Allegation Summary

  • Date Became Aware

  • Date Incident Occurred

  • Conduct Classification

  • Risk Level

The Date Became Aware field is important because statutory deadlines are calculated from the date your organisation became aware of the matter.

Select Continue to involve the parties when the required fields are complete.

Add involved parties

In Step 2: Involved parties, identify the people connected to the matter.

Complete the available fields:

  • Involved Child or Participant

  • Involved Worker or Staff Member

Use the dropdown fields to search for the relevant participant or staff member.

Select the placement type

Choose the placement type that applies to the matter.

Available options include:

  • Child Safety placement

  • NDIS participant

  • Both

  • Other

Select the option that best matches the person and service context.

Add location and source details

In the Context section, select where the conduct occurred.

Available options include:

  • On site

  • Off site

Then select the Source of Allegation.

Examples may include:

  • Direct disclosure

  • Complaint

  • Anonymous report

  • Staff observation

  • CCTV

  • Third party

  • Regulatory referral

  • Other

If you select Other, add details in the Other Source field.

Select Continue to response actions when this section is complete.

Complete response actions

In Step 3: Response actions, record the immediate actions taken.

Complete the available fields:

  • Police notified

  • Regulator notified

  • Immediate Safety Actions Taken

  • Verbatim Account

  • Full Description Of The Matter

Use the toggle fields to record whether police or the regulator were notified.

Use the written fields to provide clear, factual details.

Add the immediate safety actions

In Immediate Safety Actions Taken, describe any safety measures implemented after the matter became known.

Examples may include:

  • Removing a staff member from direct support duties

  • Checking in with the participant or child

  • Arranging additional supervision

  • Notifying a case manager

  • Escalating the matter for internal review

Keep the wording factual and specific.

Add the verbatim account

In Verbatim Account, record the exact words used by the person making the disclosure.

Do not paraphrase this field.

Only include the words that were actually said, where available.

Add the full description

In Full Description Of The Matter, provide a detailed factual account of the matter.

Include relevant details such as:

  • What happened

  • When it happened

  • Where it happened

  • Who was involved

  • Who reported it

  • What actions were taken

  • What follow-up is required

Avoid assumptions or opinions.

Submit the case

Review the details entered across all sections.

Select Submit case.

Once submitted, the system creates the case and generates a unique case reference.

The case will open in the RCS Case Workspace.

Important Notes

The case creation workflow has 3 steps:

  1. Matter details

  2. Involved parties

  3. Response actions

  • The Date Became Aware is used to calculate statutory reporting deadlines.

  • Required fields must be completed before you can continue.

  • Use clear, factual wording throughout the case.

  • The case may be created as Draft first, depending on your workflow.

  • After creating the case, you may need to use the status button to move it to Open.

Troubleshooting

  • If you cannot continue to the next step, check that all required fields are complete.

  • If the participant or staff member does not appear, confirm their profile exists in the system.

  • If the Other Source field appears, complete it when Other is selected as the source.

  • If the Submit case button is unavailable, review all required fields in each step.

  • If the case does not appear after submission, check All Cases and Drafts.

  • If you cannot create a case, check your permissions for Child Safety and Reportable Conduct.
