Add a Reportable Conduct Case

Learn how to create a Reportable Conduct case from the Safeguarding Matters area, add matter details, select the matter type, and begin the case workflow.

Open Reportable Conduct

From the left sidebar, click Child Safety.

Select Reportable Conduct.

This opens the Safeguarding Matters page, where you can manage Reportable Conduct (RCS) and Standards of Care (SOC) matters.

Review the Cases List

The Safeguarding Matters page shows all cases you can access.

You can use the tabs at the top to view different groups of cases.

Available tabs shown include:

  • All Cases

  • Active

  • Action Required

  • Closed

  • Drafts

  • RCS Register

  • NDIS Register

Each case row may show:

  • Case ID

  • Matter

  • Status

  • Risk

  • Aware Date

  • Initial Due

  • Assigned To

  • Updated

Use the Filter cases search field to find a case by keyword.

Use the All Risks dropdown to filter cases by risk level.

Start a New Case

Click New Case.

This opens the New Safeguarding Matter page.

The first screen asks what kind of matter you are recording.

Choose the Matter Type

Select Reportable Conduct.

Use this option for an allegation or conviction involving a worker under the Child Safe Organisations Act 2024 (RCS).

The other option shown is Standard of Care, which is used for concerns about the standard of care or harm to a child in care.

After selecting Reportable Conduct, continue to the case form.

Complete Matter Details

The New Reportable Conduct Matter form is divided into sections.

The first section is Matter details.

Complete the required fields. Required fields are marked with an asterisk.

Fields shown include:

  • Matter Title

  • Allegation Summary

  • Date Became Aware

  • Date Incident Occurred

  • Conduct Classification

  • Risk Level

Use the Matter Title field to enter a concise title that identifies the matter.

Use the Allegation Summary field to provide a brief factual summary.

Use the Date Became Aware field carefully, as it may affect reporting timeframes.

Select Key Dates

In the Key dates section, enter the relevant dates.

Complete:

  • Date Became Aware

  • Date Incident Occurred

The Date Became Aware field is required.

Use the date picker to select each date.

Select the Classification and Risk Level

In the Classification section, choose the relevant options.

Complete:

  • Conduct Classification

  • Risk Level

Use Conduct Classification to categorise the matter.

Use Risk Level to assess the level of risk connected to the matter.

Continue to Involved Parties

Once the required fields are completed, click Continue to involved parties.

If the button is disabled, check that all required fields have been completed.

The next section is Involved parties.

Use this section to identify the people connected to the matter.

Complete Involved Parties

In Involved parties, add the relevant people connected to the matter.

This may include:

  • Participant or child

  • Staff member

  • Worker

  • Other involved person

Select the correct person from the available records where possible.

Complete Response Actions

After involved parties are added, continue to Response actions.

Use this section to record the immediate actions taken in response to the matter.

Include clear, factual information about what was done to manage safety, notifications, or follow-up actions.

Submit or Save the Case

Review the information entered across all sections.

When the case is ready, submit it.

The case will appear in the Safeguarding Matters list and can be tracked by status, risk level, due dates, and assigned user.

Standard of Care Matters

The new workflow also allows users to create Standard of Care matters.

Select Standard of Care when the matter relates to a concern about standard of care or harm to a child in care under the Child Protection Act 1999 (SOC).

The New Standard of Care Matter form includes additional fields such as:

  • Matter Title

  • Summary Of Concerns

  • Response type

  • Date Concerns Received

  • Date Of Response Decision

  • Care Arrangement Type

  • Responsible CSSC

  • Date Became Aware

  • Date Incident Occurred

  • Conduct Classification

  • Risk Level

Response type options shown include:

  • Standards of care review

  • Harm report

Important Notes

  • Reportable Conduct and Standard of Care matters follow different workflows.

  • Select the correct matter type before starting the form.

  • Required fields are marked with an asterisk.

  • The Date Became Aware field is important because it may affect reporting deadlines.

  • Cases can appear under different tabs depending on their status.

  • Drafts may appear under the Drafts tab if a case has not been completed.

  • Use clear, factual wording when entering summaries and response actions.

  • Avoid assumptions or opinions unless they are clearly identified as reported concerns.

  • The Change matter type option may appear at the bottom of the form if the wrong matter type was selected.

Troubleshooting

If you cannot see Reportable Conduct:

  • Check that Child Safety is available in your sidebar.

  • Ask an administrator to confirm your permissions.

  • Refresh the page and try again.

If you cannot click New Case:

  • Confirm you have permission to create RCS or SOC cases.

  • Check whether your role allows case creation.

  • Refresh the page.

If Continue to involved parties is disabled:

  • Check that all required fields are complete.

  • Make sure Date Became Aware has been selected.

  • Complete any required classification or risk fields.

  • Review the form for highlighted validation messages.

If the wrong matter type was selected:

  • Use Change matter type if available.

  • If the case has already been submitted, check with an administrator before creating a replacement case.

If a case does not appear in the list:

  • Check the selected tab, such as All Cases, Active, Drafts, or Closed.

  • Clear the search field.

  • Check the All Risks filter.

  • Refresh the page.
